Most people think of SIBO as diarrhea. What they do not realize is that chronic constipation can have SIBO underneath it too. I have had SIBO twice, and both times it showed up for me as constipation.

The mechanism

Too much of a good thing, living in the wrong place.

SIBO is small intestinal bacterial overgrowth. It is too many bacteria, very often your own good bacteria, living somewhere they were never meant to be. Roughly 100 trillion bacteria belong in your large intestine. Your small intestine is supposed to have very few.

These bacteria are not the enemy. They are simply in the wrong place. That is why, when we get to treatment, I talk about pruning them back rather than killing them off.

Once they are there, they feed on your carbohydrates and ferment them, producing hydrogen, methane and hydrogen sulfide gas as inflammatory byproducts. That fermentation is also where your histamine problem comes from, because histamine is a byproduct of it.

Defense one

Stomach acid

At a pH of roughly 1.5 to 3.0, acidic enough to kill bacteria, viruses and parasites as your food arrives.

Bile

Your gallbladder releases bile salts, which help keep the small intestine relatively sterile.

Defense three

The sweep

A wave of contractions clears your small intestine every 90 minutes. Like a janitor with a broom, it only runs when you are not eating.

Chronic stress, hypothyroidism, certain medications, nerve damage, diabetes and even a single episode of food poisoning can all suppress that sweep. That is when the conditions for SIBO are set.

I received my Doctorate of Medicine from Louisiana State University Health Science Center, trained in Emergency Medicine at the University of Maryland Medical Center, and practiced as an emergency physician in Austin. But the reason I do this work is personal.

During my second year of medical school I was diagnosed with Graves' disease, an autoimmune thyroid condition. Every concern I raised was ignored or dismissed until someone finally got the diagnosis right, and then conventional medicine offered me exactly two options, both of which meant destroying my thyroid.

In 2010 I opened my own clinic, Austin UltraHealth, and became a certified functional medicine physician in 2012. Almost everyone who walked through my door had SIBO or Candida or both. I developed this protocol because the conventional treatment was costing my patients thousands of dollars, and that was not sustainable for anybody.

Before you enroll

I was told I have IBS. Is this course still for me?

It may be exactly for you. The hydrogen dominant form of SIBO is the one usually found in IBS. If you have had an IBS diagnosis for years and nothing has resolved it, nobody may have looked in your small intestine yet.

Do I need a breath test before I start?

No. You can start with the quiz. The breath test is the gold standard and it is wonderful to have real data, but not everybody is in a position to test. I teach you both paths.

Do the three steps happen in order?

No, and this is the thing people most often get wrong. All three happen at the same time. You are starving the bacteria during the same 30 days you are pruning them back and restoring your microbiome.

Why do I have to give up fermented foods? I thought they were healthy.

This is where I differ from nearly everyone else. Think about what SIBO does: it ferments things and it creates histamine. Fermented food is food for your microbiome, and if you have an overgrowth it will feed on it. I am a fan of sauerkraut and kefir, but after the protocol, not during it.

Can I just take a probiotic?

Please do not, at least not the usual kind. If you have SIBO you want to avoid probiotics containing lactobacillus, which is most of them. SIBO is a case of too much of a good thing, and adding more feeds the problem. I explain which probiotic to use instead.

I have Candida too. Do I need both courses?

This course includes a full module on treating SIBO and Candida together, because they are the two that most often come together and the protocols overlap almost entirely. You will know the exact order to add things in and the one thing to hold back.
